Patient-centered care takes into consideration patients’ individual circumstances, including:

- Other factors during care planning and implementation
It puts responsibility for important aspects of self-care and monitoring in patients’ hands, along with the tools and support they need to carry out that responsibility. In this way, patient-centered care can reduce unneeded and unwanted services and also improve outcomes.
